Job Title: Community Health Worker/Patient Navigator

Location: School Based Services Central

Department: School Based Services

Employment Type: Full-Time

Shift: Mon-Fri, Day shift

Job Summary: Facilitate communication and patient empowerment in interactions with healthcare/social service systems. Troubleshoot barriers to adhering to care. Engage the community through outreach to recruit and engage high-risk patients. Serve as a liaison between health/social services and the community to facilitate access and improve service quality/cultural competence. Assist the care team in developing care management processes. Build individual and community capacity through education, support and advocacy. Collaborate with patients/care teams to overcome barriers to healthcare goals. Connect patients with resources and services.

Key Responsibilities:

  • Facilitate communication and patient empowerment with healthcare/social services

  • Troubleshoot barriers to patient adherence to care

  • Engage community through outreach to recruit/engage high-risk patients

  • Serve as liaison between health/social services and community

  • Assist care team in developing care management processes

  • Build individual and community capacity through education, support, advocacy

  • Collaborate with patients/teams to overcome barriers to healthcare goals

  • Connect patients with community resources and health center services

  • Follow-up with patients via calls, home visits, other settings

  • Conduct public education on availability of qualified health plans (QHPs)

  • Distribute impartial QHP enrollment information

  • Facilitate QHP enrollment by assisting with options, eligibility, forms

  • Provide culturally/linguistically appropriate information

  • Other duties as assigned

Education and/or Experience Qualifications:

  • Bachelor's degree in related field preferred; High school diploma required

  • 2+ years experience in community health, human services, healthcare preferred

  • Desirable experience: customer service, patient/peer coaching, medical interaction, community relations, outreach, data entry, public speaking

Knowledge, Skills, and Abilities:

  • Knowledge of harm reduction, motivational interviewing, treatment modalities

  • Knowledge of local community culture and values

  • Knowledge of customer service principles and processes

  • Ability to listen attentively and understand others

  • Ability to communicate information and ideas clearly

  • Proficient in Microsoft Excel

  • Excellent verbal and written communication skills

Supervisory Requirements:

  • None

Employment Requirements:

  • Evidence of being free from infectious/contagious disease

  • TB test

  • Successful criminal/child abuse background checks

  • Complete New Hire Orientation within 30 days

  • Complete all training requirements including Relias within 2 weeks

  • Achieve 36 hour training requirement per regulations (MO staff)

  • First Aid certification within 3 months

  • CPR certification within 3 months

  • Behavioral management training within 30 days

  • Complete "Direct Service Expectations" within 30 days

  • Extensive knowledge of policies, procedures, regulations for assigned program

  • Valid driver's license in state of residence

Physical Requirements:

  • Sedentary work - exerting up to 10 lbs of force occasionally

  • Primarily sitting, with some standing/walking briefly

  • Meets criteria for sedentary work per ADA

For the FQHC Foster Care portion:

Job Title: Community Health Worker (Foster Care)

Key Responsibilities:

  • Assist foster parents in accessing medical, dental, behavioral health care for children

  • Conduct SDOH screening using PRAPARE Assessments for children/foster families

  • Provide care coordination - facilitating transitions, referrals, follow-up

  • Make referrals and connections to community resources for basic social needs

  • Other duties as assigned

Education and/or Experience Qualifications:

  • Preference for CHW with lived experience:

  • As a child in MO foster care

  • As a parent in MO foster care

  • Adopted from foster care

  • Worked/volunteered with child welfare, advocacy, justice, behavioral health org

  • Bachelor's degree in Social Work, Psychology, Public Health, Health Ed or related

  • 2+ years experience in community health, human services, healthcare preferred

Knowledge, Skills, and Abilities:

  • Same as general CHW/Patient Navigator role

Employment Requirements:

  • Evidence of being free from infectious/contagious disease

  • TB test

  • Extensive knowledge of policies, procedures, regulations for assigned program

  • Valid driver's license in state of residence
