Trinity Health Manager Nurse Navigator FIND Program-Remote in Livonia, Michigan

ESSENTIAL FUNCTIONS

  1. Knows, understands, incorporates and demonstrates the mission, vision and values of Trinity Health in leadership, behaviors, practices and decisions.

  2. Responsible for the daily oversight of regional FIND/LCS colleagues to optimize operations through standard workflows and processes. Responsible for escalating potential opportunities for workflow improvement as appropriate.

  3. Participates in the recruitment and selection of colleagues for the FIND/LCS program.

  4. Responsible for payroll review, PTO approval and coverage.

  5. Participates in regional staff evaluations.

  6. Ensures all patients that are identified with incidental and/or unexpected findings with recommended follow-up are contacted and offered the option of the recommended follow-up.

  7. Provides personalized nurse navigation interventions to patients who request additional information, assessment and/or referral. This includes assessment of patient educational level, barriers to learning and other psychosocial factors that may impede remarkable care.

  8. Responsible for ensuring that the regional navigation team is optimizing physician time by ensuring that appropriate follow-up intervals are recommended for incidental findings and that duplicate follow-up requests are avoided.

  9. Supports provider decision making by:

a. Assisting PCPs in ensuring their patients get appropriate follow-up for incidental findings and lung cancer screening at intervals specifically recommended by the radiologist.

b. Obtaining additional imaging for incidental findings and lung cancer screening only at the specific request of the PCP. These patients are then provided education and navigation through the follow-up process.

c. Providing referral and education based on care deemed appropriate.

d. Directing patients to the correct provider based on the recommendation for follow-up of incidental findings.

  1. Utilizes the available software programs to track daily/periodic assignments.

  2. Maintains frequent communication with local and regional colleagues involved in the FIND/LCS program as well as the Radiology and Oncology departments to facilitate the development of the program and maximize patient care.

  3. Functions independently while remaining calm in a high-stress environment. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions and participate in their resolution.

  4. Performs other duties as assigned.

  5. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical, and professional behavior

MINIMUM QUALIFICATIONS

  1. Graduate of an accredited school of Nursing and Bachelor of Science Degree in Nursing.

  2. Five (5) to seven (7) years of progressive clinical nursing experience is required

  3. One (1) year managerial/leadership experience preferred

  4. Current license to practice as a Registered Nurse in the State of Michigan. Additional state-specific licensing may also be required.

  5. Problem solving skills and ability to initiate creative solutions to problems.

  6. Demonstrated ability to expertly navigate a variety of software applications.

  7. Effectively communicate with Medical Staff, departmental staff, Trinity Health department representatives, physicians, other health providers and facilities.

  8. Working knowledge of Radiology procedures protocols and processes, preferred.

  9. Knowledge of Windows based programs.

  10. Effective organizational skills.

  11. Must possess a personal presence characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health
